In case you have lost your Life Membership Certificate or your ID card hard copy, you may send an email to secretary@aiota.org with the necessary documents (Life membership certificate copy and declaration letter stating that you have lost your certificate with your life membership number. Sending ACOT Certificate will not be valid.) . Soft copy of your certificate and ID card will be generated and you will be able to download the same from the AIOTA website using your login credentials that will be sent to your mail within 3 -7 days.
There is a fee of Rupees 800/- for the same.

Once the payment for Life Membership is done, you will receive your login credentials for the AIOTA website in your mail (check your spam messages as well). It can take around 10-12 days. You can then log in to www.aiota.org using those credentials and download your certificates and ID card online.

You will be asked to upload the following: Internship completion certificate, University degree or provisional certificate, colour passport size photo and ID/Address proof.

Due to our confidentiality policy, we are unable to provide you with copies of completed forms/documents or email copies relating to your license verification. We will however update you via your registered email once the procedure has been completed. Good standing certificates will be forwarded to your email on a case-by-case basis.

AIOTA Life Membership will be only given to those who have taken the degree from AIOTA accredited colleges. The AIOTA life membership validity is 15 years. After 15 years, the membership has to be renewed. Membership registration and payment can be done only through online mode through AIOTA website(No DD /cheque/NEFT etc)

1. Go to www.aiota.org and on the top of the page, click on "Apply Life Membership Online"
2. You will be directed to "Life Membership Application Form".
3. Please provide a Gmail address for email contact while completing your form. We are unable to send login credentials to any other email address other that Gmail.

5. Fill the form using the necessary details and you will be asked to upload the following: Internship completion certificate, University degree or provisional certificate, colour passport size photo and ID/Address proof.
6. Now you will be making the payment for the same via net banking using your credit card/debit card/using your UPI ID; costs Rupees 8500/-
7. Once the payment is made, your Life Membership certificate, IOTR Certificate and ID card will be generated within 10-12 days(only soft copy). Once the payment is done, refund will not be made.
8. You will receive your login credentials for the AIOTA website in your mail (check your spam messages as well).
9. You can now log in to www.aiota.org using those credentials and download your certificates and ID card online.
10. Hard copy of the certificates and ID card will not be sent.
11. The overall registration process can take around 10-12 days.

If you completed your under graduate degree in Occupational Therapy from a college that was accredited during your course of study and if your college has since then been denied the accredited status and you wish to obtain a life membership certificate, please ensure that you have mailed the Dean of the academic council of your intension on obtaining a life membership at deanacot@aiota.org and have forwarded a copy of the mail to the treasurer at treasurer@aiota.org.

Please attach scanned copies of your educational certificates so that they may be verified.

Once you have been approved by the Dean to obtain a life membership and have made the necessary payments to AIOTA, you may proceed to mail the secretary at secretary@aiota.org for obtaining your life membership certificate.
